Student Post-Surveys
Please click on the grade level below to begin taking the survey. All student surveys must be completed by Friday, February 12.
Keep in mind, this is simply a survey to provide benchmark data following kit implementation. All data collected is the property of the Kansas City Science Initiative and is used only for our programmatic purposes. Any information shared with your school district is provided in a composite format so that no teacher or student is individually identifiable.
Teachers: Please note that both your JDE number (employee number you use for daily attendance) and the student's district identification number must be entered at the beginning of the survey.
There is one question per page and each question must be answered before moving on to the next.
At the end of the survey, click on the "Submit" button and your survey will be received as complete.
